    • The information I have provided in this application will be used by Healthy Boost to evaluate my suitability for a Juice Bar Kiosk license.

    • Submission of this application does not guarantee approval or the issuance of a license.

    • I understand that a $350 refundable application fee is required to process this application.

      • If my application is not approved, the $350 fee will be refunded in full.

      • If my application is approved, the $350 fee will be applied toward the $125,000 licensing fee.
